Consent to Use Photographs

I, the undersigned, hereby grant Tiffany Loyola of Luminous Lounge LLC and its agents, employees, and assigns the irrevocable and unrestricted right to use, publish, and copyright my likeness in photographs and/or video recordings taken during or following my spray tan service, for any lawful purpose, including but not limited to promotional and marketing materials, social media, advertisements, and websites.

I understand and agree to the following terms:

Usage: Photographs and/or video recordings may be used in any manner and media, including but not limited to print, digital, or online platforms, and may be altered or edited at the Business's discretion.

No Compensation: I acknowledge that I will not receive any financial compensation for the use of my images or likeness.

No Obligation: I understand that the Business is not obligated to use the images or recordings, and my participation is voluntary.

Revocation: I may revoke this consent at any time by submitting a written request to Luminous Lounge LLC, but revocation will not apply to any uses of my images or likeness that have already occurred prior to such revocation.

Release of Liability: I release Luminous Lounge LLC from any claims, demands, or causes of action that may arise from the use of my photographs and/or video recordings as described above.

Confirmation: By signing this form, I confirm that I am over the age of 18 (or have parental consent if under 18), and I agree to the terms and conditions outlined above.

Service Acknowledgement:

I, the undersigned, hereby acknowledge that I am voluntarily requesting a spray tanning service. I understand that the solution used contains DHA (dihydroxyacetone) and other ingredients, and I have been informed of the possible effects and results of the treatment.

Health Disclosure:

I confirm that I do not have any allergies to the ingredients in the tanning solution. I also certify that I am not currently undergoing any treatments that may cause adverse reactions to spray tanning, including but not limited to:

Skin conditions (e.g., eczema, psoriasis)

Allergies to skin care products

Recent surgeries or skin treatments

Risks Acknowledgment:

I understand that while spray tanning is generally considered safe, there may be risks, including but not limited to:

Skin irritation or allergic reaction

Uneven tan

Staining of clothing or bedding

Release of Liability:

I hereby release, waive, and discharge Luminous Lounge LLC, its owners, employees, and agents from any and all claims, demands, or causes of action arising out of or related to my participation in the spray tanning service.

Consent to Treatment:

I consent to receive spray tanning services and agree to follow the aftercare instructions provided by the technician. I understand that if I have any concerns about the service, I will discuss them with the technician prior to the treatment.

Requirements:

You will be guided to the airbrush tanning area where you will dress down to your comfort level. Nudity is allowed if you are 18+ or have parental consent. Disposable undergarments are available upon request. Understand that the solutions bronzer will wash out of most clothing.

All cancellations must be done 48 hours+ in advance in order to avoid penalties. This allows me to accommodate other clients and schedule accordingly. Clients who do NOT SHOW up for their appointment or cancel in less than 48 hours, will be charged the full service price. If you need to RESCHEDULE, please contact me as soon as possible, I will do my best to find a new time that works for you. To confirm your appointment, a valid credit or deposit will be required. This card will be used for any applicable cancellation fees. All deposits are NON REFUNDABLE if you fail to comply with our cancellation policy. If you do not pay a deposit, the full amount will be charged for your service that is booked. For your convenience, all appointments can be canceled or rescheduled through your email confirmation or on the scheduling app.

Parties of 4 or more will have to cancel 2 weeks prior. If anyone in your party cancels last minute, the remaining balance will still be at your discretion.

My mission is to provide the best spray tan possible. Here's how you can help!

For best results: Exfoliate, shower and shave 24 hours before your appointment. When exfoliating, focus on areas prone to dryness. (Elbows, knees, and ankles.) If showering the day of, make sure it is at least 4 hours in advance. To remove any previous spray or self-tanner, use an exfoliating glove, dry brush or tan remover.

Make sure to do any massages, manicures, pedicures, waxing, and facials at least 24 hours prior to your scheduled appointment. Make sure to remove ant residual oils before your appointment.

Book your appointment 1-2 days prior to any important event or vacation.

Clean/dry skin is the best to ensure the longevity of your tan, don't apply moisture, deodorant, makeup, or oils before spray tanning and avoid using any body scrubs, or scented products like Bath & Body or Victoria's Secret, as they can create a barrier.

Bring loose, dark colored clothes to change into after your spray tan to be most comfortable. Avoid tight-fitted clothes that can rub off the tan.

After Appointment:

Instructions cards for post-care will be provided.
